Partnerships for Health formally concluded on March 31, 2011, however the learnings from this successful quality improvement initiative will be carried forward through Partnering for Quality!

Partnerships for Health (PFH) was an innovative South West LHIN initiative that engaged health care teams to transform the way they provide care to those with chronic disease. Using diabetes as a proxy, primary care and community care practitioners undertook a structured process to implement Ontario’s Chronic Disease Prevention and Management Framework using an inter-disciplinary, team-based approach. Central to this initiative was a partnership between the family physician and CCAC case managers whose role is to assist the individual with diabetes in navigating the health care system, accessing community resources and linking to specialist services and tertiary care when appropriate. The results were amazing with statistically significant improvements.
